HOW IS MY PRIVACY AND HEALTH DATA PROTECTED?

The personal information that you provide to GP2U/PSYCH2U is used and disclosed only for the purpose of confirming your eligibility and delivering and evaluating the program. If you don't provide this information, GP2U/PSYCH2U may not be able to provide you with the services which form the program.

GP2U/PSYCH2U won't disclose your personal information to HCF other than your name, date of birth, membership number and date of participation with the program for the purposes of checking your eligibility and for billing and reconciliation. With your consent GP2U/PSYCH2U may disclose your personal information to your regular doctor or other healthcare provider.

Why does GP2U/PSYCH2U collect health information?

GP2U/PSYCH2U will only collect health information that is necessary to provide a health service, respond to your enquiry, complaint, or requests, provide self-assessment reports and to adequately co-ordinate and manage the provision of products, tools and services for your use.

How will health information be used?

GP2U/PSYCH2U will only use collected health information for the purposes of:

GP2U/PSYCH2U will not use any collected health data for any purpose unrelated to the purpose for which you provided it. Only de-identified aggregate data will be used in reporting sent to HCF for the purposes of evaluating the service; no individuals will be identifiable.

What would happen if we did not collect your personal information?

It is your right not to give GP2U/PSYCH2U any personal information, request your information not be disclosed and not to identify yourself. However, if you choose not to disclose your personal information, or do not identify yourself or ask us not to use or disclose your information for a particular purpose, GP2U/PSYCH2U may not be able to offer our health services to you.
